    Prehabilitation

    it is the process of enhancing an individuals functional capacity to enable him or her to withstand a forthcoming surgery. It is a multi modal approach that includes.

    • Medical optimization- preoperative smoking cessation, reduction in alcohol intake, and weight optimization benefit the patient in post operative period. These helps in reduction of risk of cardiopulmonary complications, wound infections, impaired healing of wound or bone, prolonged hospitalization.
    • Physical exercise- a baseline assessment of functional capacity is performed using cardiopulmonary exercise test or 6 min walk test. And incorporating a suitable exercise programme and using the same tests to measure any changes in functional capacity. A combination of strength and aerobic exercises are included in programme as both muscle and cardiopulmonary fitness decline post surgery. Exercise intensity is optimized using heart rate monitoring or the use of borg’s scale. This all is well structured and tailored as per individuals preference and conditions where it enables you to see the progress.

    • Nutritional support- literature has clearly shown that poor nutritional status is associated with poor postoperative outcomes; length of stay, infectious complications, readmission rates etc. being anemic, underweight or overweight (malnourished) or the grade of surgery should be considered. Interventions include preoperative carbohydrate loading reduces insulin resistance, minimizes protein loss, lean body mass and muscle function. Also some evidence in support of immunonutrition i.e. ingestion of amino acids, omega 3 fatty acids, and nucleotides promotes wound healing, reduces infectionrates, shortens length of stay. Such regimes are optimally commenced 5-7 days prior to surgery and continued post surgery for similar period in malnourished.
    • Psychological support- the causes of fear in the patient awaiting surgery is manifold. The underlying diagnosis, surgery, anaesthesia, pain, survival, and recovery are all causes for concern. These stressors produce harmful effects on body. Aim of psychological support is two;
      • To reduce psychological distress and anxiety associated with diagnosis and surgery.
      • to maximize patient’s motivation and empower them to comply with exercise and nutritional aspects of the programme
    Post Operative Orthopaedics
    • Ligament/ meniscus repair: common surgeries are ACL(anterior cruciate ligament) reconstruction , medial meniscal repair done in athletes as well as normal population. Physiotherapy rehab helps you regain muscle strength, joint position sense, and good balance. It helps transitioningback to your active lifestyle. Without complete rehab there is a increased risk of reinjury or developing muscle insufficiency.
    • shoulder surgeries: common shoulder surgeries are done post shoulder fracture, dislocations, rotator cuff repair.
